Keto doesn't seem to be working for me this time? Any women with PCOS able to offer advice?

THE QUESTION

I've (32F) done keto 4 times in the past and it's always been super effective for me. This time I am seeing literally zero results and wondering if could be related to a diagnosis I just received? The first time I did it was for 8 months in 2014 and in conjunction with the gym, I went from 162 to 128 lbs. The second time I did it was because I remembered my energy was higher during keto. I didn't monitor my weight at the time because I was already at my goal weight of 123 lbs. I don't know if I lost weight. That time I did keto for about 6 months as well. Then in 2019 I had gain some weight after a bad breakup and career shift. I'd been stress eating and shot back up to about 170 lbs so I did it for 4 months and lost 25 lbs.
